Rates, Forms, and Insurance

Rates are set based on my level of experience and training. These rates reflect the investment made to provide you and your family with the best treatment possible as well as my commitment to consistently building my skills and experience.

I have advanced training and specialized certifications in areas such as Play Therapy, Sand Tray, and provide Supervision and Peer Consultation.

Rates

$325 Intake Session with parents

​

$235 per 45- minute in person therapy session

​

$100 per 50-minute face-to-face or distance supervision or consultation session. Special rates for groups

​

Hourly rate on Phone, Video, Email Consulting, Supportive documents, writing letters or reports

​

**No Show Fee (session fee) if cancelation is not received 48 hours in advance.

Insurance

Services may be covered in full or in part by your health insurance plan, depending upon your health plan's mental health services coverage. Services are considered out-of-network with most insurance plans. At your request, we will provide a statement of services or superbill for you to submit to your insurance company for possible reimbursement. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits (jn-network and out-of-network)?

• What is my deductible and has it been met?

• How many sessions per year does my health insurance cover?

• What is the coverage amount per therapy session?

• Is approval required from my primary care physician?

 

Payment

Cash, check and all major credit cards accepted for payment.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 48 hours in advance, you will be required to pay the full $235 for the missed session.

Forms

​

Consent forms, Release of Information, Out-Patient Services Agreement, Good Faith Estimate, COVID screenings will be provided through the patient portal.
